Semester Break Form
This form is for students who have not registered for Summer or Fall 2012 classes and want to have access to library resources during the summer. This includes borrowing IIT materials, on-site borrowing at I-Share libraries, document delivery and interlibrary loan. Privileges granted through this form last from May 6 - August 19, 2012.
If you are unsure if your library privileges are current you can check with the Circulation department or stop by the Circulation Desk on Galvin's lower level.
By submitting this form you agree to be responsible for any fines, fees or replacement charges for damaged or lost materials; to abide by the IIT code of conduct; and adhere to the circulation policies and procedures of Galvin Library, including returning borrowed materials on time and the library's E-Resources & Computers policy.
